CAMPERSHIP APPLICATION

Step 1. Fill out our Campership Application

Step 2. Please email a pdf of the child/applicant's most recent school grade report to RuthAnn@TheEagle.Foundation

Step 3. Please email your child/applicant's IEP or 504 to RuthAnn@TheEagle.Foundation (if applicable)

Step 4. Please obtain a character reference for the Camper. The reference must be provided by a teacher or school counselor. The character reference can be completed in one of two ways:

1. Name the character reference below and email him/her a link to a digital form to complete online (preferred)

2. Print out, complete, scan, and upload the character reference using the upload tool below. To print a paper copy of the Character Reference Form, click here. Please note, you will need to have the completed Character Reference ready to upload before you begin this application.

Step 5. Submit a Camper Statement Video to help us get to know your child better.

Step 6. You must sign a release for your child's school to provide information about your child's eligibility for free or reduced price school meals. Your family does not need to participate in the program to be eligible for a Campership, but your family's eligibility is a significant determining factor. The release will allow the school to communicate this information to us.

If you have extenuating circumstances that disqualifies your family from the free lunch program, you can upload a statement of such.
